Memberships

This Agreement is effective as of the date signed and will continue for a 12-month term, with monthly payments drafted automatically from the card on file.

At the end of the 12-month term, the membership will not automatically renew. To continue receiving membership benefits, the Member must sign an agreement and consent to renew for a new term.

Cancellation Policy

• This membership may be canceled before the 12-month term is completed with 7 days notice prior to the next billing date.
• If no services or products have been rendered or used, the Member will receive a full refund of any payments made and no cancellation fee will apply.
• If the membership is canceled after services or retail discounts have been used, the total monthly membership amount paid will be applied towards the full retail price of services rendered.  
  • For example, if 75 units of Dysport are administered and the member has paid two months of membership dues at $116 per month, the total dues paid ($232) will be credited toward the retail cost of the treatment ($337.50). The remaining balance of $105.50 will be automatically charged to the card on file. 

Late Fees

• Payments 1–2 weeks late: $50 late fee
• Payments 3–4 weeks late: $100 late fee
• Payments more than 4 weeks late: Membership will be canceled, and the remaining balance for any services already received under the membership will be charged to the card on file or forwarded to collections if necessary.
